Rehabilitation-
Fairacres Manor contracts with Rehab Care to provide
physical, occupational, and speech therapy in our spacious and modern
rehabilitation gym. Our rehabilitation program focuses on therapy and
re-education in functional activities including wheelchair transfers and
mobility, gait training, acuities for daily living, improvement in endurance
and fitness levels and cognitive skills. The therapy staff also provides
free therapy screens to new residents, as well as, home visits for any
resident contemplating discharge.

Subacute
Care -
A comprehensive short-term care designed for residents who have
a severe illness, injury, worsening of a disease, or who need specialized
rehabilitation following hospitalization. Opened in January 1998, Fairacres
Glenmere Subacute Center offers transitional care between the hospital
and home. A team of professionals evaluates each resident's condition
and an individualized plan of care is developed and followed. Care conferences
are held to monitor the resident's progress and revise the plan of care
as needed. Individual resident's conditions and medical needs vary widely;
therefore, the length of stay on Glenmere may range from several days
to several months.
The Glenmere Center's staff is highly experienced in
caring for residents who no longer need hospital-based services, but still
require specialized, coordinated treatments for complex medical problems.
Physicians make regular rounds, and the unit is staffed by Licensed Nurses
24 hour a day.
Oxygen, suction, televisions, and telephones are provided
with each room on Glenmere. Visitors are always welcome!

Care -
Fairacres Manor is dedicated to providing our community with an
enrichment philosophy of care for individuals with Alzheimer's and related
dementias. Our mission is to ensure safety and security while encouraging
functional independence through meaningful day-to-day activities.
Alzheimer's is a progressive disease, however, the rate
of progression varies with each case. The Homestead Center provides a
setting catered to the unique needs of all of our residents. Residents
who may benefit from the safe supportive and structured setting are carefully
assessed before being admitted to Homestead. Our staff is specially trained
to recognize the stages of this disease and to interpret changes in the
resident condition. A formal assessment of each resident is conducted
every three months or more often if necessary. An experienced outside
reviewer ensures that each resident meets criteria so that admission and
continued stay on Homestead is appropriate.
A secure outdoor walkway and patio area are integrated
into the unit so that those
residents who enjoy the outdoors may do so safely. Other common areas
are designated for dining, activities, or quiet relaxation. Within the
Homestead's safe environment, staff creates an atmosphere of supervision
rather than restriction.
Each day is carefully tailored to meet the special needs
of the residents, as well as, accommodating individual preferences. Small
group and one-on-one activities within Homestead maximize independence
and increase socialization of our residents. Residents are also encouraged
to join in activities held in other areas of Fairacres Manor.
